Jack, age 90, of MPLS. Passed away peacefully on August 10, 2019. Preceded in death by parents, wife: Phyllis, son-in-law: Rick Forsgren and great-grandson: Kody Thorn. Survived by sister: Gloria, daughters: Jackie and Joyce, stepchildren: Wayne (Donna), Keith (Diane), Jeanette, Shawn (Linda), Maureen (Bill), Julie and Mark, 15 grandchildren, 21 great-grandchildren, 1 great-great-granddaughter, and many nieces and nephews. Thank you to Linda and Keith for bringing Gloria to see Jack. A heartfelt thanks to all caregivers from Legacy that took care of Jack with special thanks to Amina. Your care allowed us to grant Jack his last wish….to remain at home. A very special thank you to his hospice team…Chris, Abby, Andrew etc. You are special angels and our family will be forever grateful for the care and self-respect you gave to Jack. A Celebration of Life Wednesday – August 14th, 2019 at KOZLAK-RADULOVICH CHAPEL 1918 University Ave. NE at 11 AM with visitation one hour prior. Luncheon immediately following services. Private interment Sunset Cemetery. Memorials preferred to Sacred Heart of Jesus Polish National Catholic Church.
